The Connections program serves Sacramento and provides housing and services to keep formerly homeless young people ages 18-24  from returning to homelessness. Individualized case management focuses on acquiring the life skills needed to become self-sufficient. Approximately 16 youth live in rent-subsidized apartments in a variety of locations and receive up to 24 months of individualized case management.
